The Region II EMS Directors Association awards are designed to recognize individuals who have made noteworthy contributions to EMS.  The awards will reflect both past and present contributions.

The Region II EMS Director’s Association’s Service of the Year Award will honor an EMS Service which is a member of the Region II EMS Director’s Association.

Administrator of the Year
Lifetime Achievement Award

Submit supplemental information such as: recommendations or support letters, newspaper articles, photographs, and/or any other information which will help the members of the Awards Committee with their decision.

INFORMATION TO INCLUDE FOR:
Service of the Year Award

A computer generated or typewritten nomination containing:

Service name, address, and phone number
Give an overview of the organization, such as:
Date organization began service
Level of service
Number of personnel and units
Annual operating budget
Population of area of service
Describe any awards received by the service
List any community involvement.
New programs implemented
Submit supplemental information such as: newspaper articles, letters of support,
photographs, and/or any other information which will help the members of the Awards Committee make their decision.

THE COMMITTEE WILL ACCEPT NOMINATIONS UNTIL THE ADJOURNMENT OF THE REGION II MONTHLY MEETING ON SEPTEMBER 25, 2012. NOMINATIONS RECEIVED AFTER THE APPOINTED TIME WILL NOT BE CONSIDERED.
